Their license and every notice about it go to this address. It is shown again before you pay, so a typo is fixed here for free.

Annual Monthly

If your state charges sales tax on software, it's added in the payment window. You'll see the total before you pay.

Using PatientPapers on more than one computer? Your computers don't share anything with each other or with us, so each one renews its license on its own with one press. On the annual plan that's once a year per computer. On the monthly plan it's once a month.

I will review every form for accuracy and appropriateness before signing, and every certification, signature, and statement I produce with PatientPapers is my own professional act and my sole responsibility. What PatientPapers is not. PatientPapers is a documentation tool. It does not provide medical or legal advice, does not decide FMLA or benefit eligibility, and does not guarantee that any form will be accepted by any employer, agency, or insurer. Agreement. I am a licensed healthcare provider — or authorized staff purchasing on behalf of one — and I agree to the Terms of Service and Privacy Policy.

Why we ask: these confirmations protect your patients and make clear the clinical judgment on every form is yours — the tool never signs anything.

Can I email you occasionally to ask how it's going? (optional)

Getting the license. Payment mints a license key for the provider you named. It is shown on the next screen and emailed to them, and it stays the same for as long as they subscribe. Each period they use it on the license page to get a fresh license, and we send one by email too.

Nothing you enter in the app reaches us. Patient answers, signatures and finished forms are stored on your device. We have no copy, so there is nothing on our side to breach, subpoena, or lock you out of.

Cancelling. Cancel any time from the subscription-management page linked in every receipt Paddle sends you, or by emailing support@patientpapers.app. Cancelling stops the next charge; the license already held runs to its printed expiry date.

Renewal reminder. Before an annual renewal we email at least 30 days in advance, stating the price and that sales tax may be added.

Payment. Our order process is conducted by our online reseller Paddle.com, the Merchant of Record for all our orders. Paddle collects the card; we never see it.
